Requesting for copy of application form

Hi
I am trying to enroll my kid to a childcare centre when he turns 18 months. There is the standard application form to be filled up and submitted to the centre. On 2 pages of the form, applicants are required to make a declaration on a few items, eg Agree to allow child to attend indoor and outdoor activities organised by the centre, agree to allow centre to use child’s photo/video for publicitiy purpose.
The point is, my request to have a copy of the submitted application form was denied. I wonder how would parents know what they have agreed to in the declaration section if they are not allowed to keep a copy of the application form?
Do you have the same situation for your child’s childcare registration?
The centre that I applied for is located in Clementi.

Yes, I was shocked too.
The reason was that the form belongs to the centre and is for the centre's safe keeping. There was no precedence and I am the first parent asking for a copy. -

Calling the HQ/main office to clarify. HQ/main office indicated that although there is no black/white on releasing of application forms, it should not be a problem in releasing this.
Maybe the centre is new and is still learning.
I am looking forward to 'engaging' the centre's principal and evaluate the centre's attitude in accepting parent's feedback.
Will update soon.
I wonder if there are other centres that refuse parents of the copy of the application form, or there are parents here whom have gotten a copy of the form.

to my, a childcare's flexibility in accomodating a parent's request is most valued.
What we loved about our DS's former childcare is that they would try tried their best to accomodate our request, and they would act on our feedbacks.

sometimes the best systems downfall caused by 1 or 2 rigid staff who have some strange ideas. anyone with half a sense should know that is ok to give the parent a copy what.... :?

update:
called the principal and my impression is that she does not accept parent’s feedback with open mind.
I was misunderstood several times and I am now painted as ‘bad guy’, unfortunately.
With an open heart and mind, I am trying again. Will be speaking with the principal in person during lunch today. Hopefully from body language and facial expressions, we could communicate better.
