      Hi Leanne, what due date on Apr 6th are you referring to?

      Also, my girl was not born here so doesnโ€™t have health booklet and also all her immunizations have been given in birth country. so what will we need to provide? Is there a list of all immunizations so i can get a certificate from her pd in birth country?

        The due date is for ds1 to submit ds2 application directly to school, as ds is currently studying in the same pri school.

          must produce immunisation cert upon registration of P1? how to obtain. Isn't just photocopy of health booklet is good enough? :?:

            Photocopy of the relevant page from the health book is fine, too.

                May I know what kind of immunisation cert u submit since you missed out one jab?I dont think missing out one jab will deny the child for application into the school right?

                In my son's case as he is the 2nd child, we applied under phase 1. I think I photocopied the page on his health booklet that shows the injection that he has taken so far which has the paed's stamp on it.

                BTW, he don't have a certificate unlike my older and younger girls. Subsequently the nurses went through that to ascertain that he missed out 1 jab.

                I don't think the school will deny registration due to the fact that a child miss a jab since he has taken 95%. That is required more from health monitoring by HPB. Also my impression is that some parents might defer/ prefer their child not to take some jabs. The nurses actually call me for consent before they administer the jab.

                  Sorry - sidetrack abit.


                  I recently received a letter from MOH telling me that my 3rd ds 18mth jab was not done yet (he is 2.5yrs now) and i am supposed to revert to them once the jab is taken.

                  So - i think they actually have ways of identifying if you have completed the compulsory jabs or not, way before registration. ๐Ÿ˜‰

                    i requested a copy of the cert for my DS & got it in my mail within 2 working days.

                    or if u want, u can also print out a copy from the above website directly. ๐Ÿ™‚

                        When we registered for DS 2 years ago, the school didnโ€™t check the immunization record at all.
