      hee, i think to be KS a bit, bring both the health booklet and immunizations cert. then see which one the admin want to take.

      To let know you the extent of hubby's KSness, he brought along both the health booklet and the printed out copy (from SingHealth). Also photocopied the birth cert,our IC and my PSLE cert (Phase 2A2) as well.

        Not sure if I got the responses wrong, but where there is a discrepancy between central records and your own doctor signed version, it could be that your doctor did not inform MOH.


        As long as you have that cert that is signed and chopped by doctor, MOH will recognise. Just call up the relevant dept. YOu may have to fax over that cert for their verification.

          I have same problem w the MOH printout not having all the records and called MOH. Am told there is some problem with their IT system and that it is sufficient to bring the doctor’s certificate for P1 registration.

            hi all,


            As long as the certificate bears the dates of immunization and signed/chopped by doctor, it’s ok. They only want to make sure your child has received all the immunization - not bar one from applying to Pri school.

              Do bring along the immunisation booklet...although very messy, with the jab sticker and doc clinic stamp, it's always safe than sorry. Last year, the admin staff scrutinised every single jab :faint:

                so if the cert does nt bear all the required immunizations, partly becos maybe the doc did not update HPB accordingly,

                but our health booklet did indicate, sign & chopped by the doc, still gt pa kei or nt ... :sad:

                worry now... do we hv to call the HPB to verify :?

                  yes, if doctor signed and chopped and put down the date, you are safe. They won’t log onto hpb to verify if hpb’s records were updated.

                        For those on phase 2C and 2C supplementary, the registration dates indicated on the press release (26 to 28 July) and Annex A (27 to 29 July) are different. I’m a bit confused which one is the correct date.
