IF you are referring to hokkien huay kuan form, you can download from website. http://www.shhk.com.sg. Go to About Us, then click Join Us. The terms & conditions are available for download at the same page.

Thanks for the reply hquek, but I was looking for the Phase 2B recommendation Form from SHHK. I heard that it has a proprietary format. I'm already a member of the huay kuan. I tried asking for the form but was asked to wait until next year. So if anyone out there has the form from this year, please let me know. Thanks in advance

Hi, I have enquired with the schools directly.


For CHIJ TP, we just need to bring the parent’s baptism certificate at the time of phase 2B registration. However, for Maris Stella High, the child himself need to be a baptised roman catholic before he can qualify under phase 2B.

Hence, depends on which school you are interested in, you have to enquire with the school directly.

If I am correct, the format should be the same, they just use different colour form every year to differentiate the year u applied.

Hi diamond_shortcake, have you checked the earlier pages of this thread? Generally, if you are a member of a church/organization of the faith of the school, chances are good that you can get priority for your child.
