ETA: Although, I would prefer to put the main clause at the end, thus:

Found this vocabulary cloze question which is quite tricky.
Passage: I was going on one of my rounds when l spotted an elderly woman attempting to sit up on her hospital bed. She looked (1) [thin and sickly] as she lay back on the hospital bed, exhausted by her failed attempts to do so....
1. Choose the word(s) closest in meaning to the words \"thin and sickly\".
(1) lanky
(2) pallid
(3) gaunt
(4) scrawny
Believe most pupils and even parents have to check dictionary to find the answer. But even with the dictionary, two of the answers appear to be quite similar. What do you think?
