      I would advise you to print out that copy of your daughter's immunisation record from NIR. It's so much clearer and neater too. Even the lady who attended to me last week said so. The records in the health booklet can be rather messy with all the smudges, stickers of immunisation jabs, stamp and signature of doctors on the page.

      I've printed the one from NIR, but in NIR itself, there's 2 pages right? 1 is summary and the other is chart form..?? Just bring these 2?

        Hi autumn,

        If record in NIR shows 2 pages, then bring both copies along. Tis is the time to bring everything relevant. 1 copy less and you'll be banging your head on the wall. :stupid: My dearest hubby checked, checked and re-checked all the documents to bring. In the end, my report book wasn't required, just the PSLE cert.

        BTW, the staff member checked all the jabs my daughter had been given at birth...the copy which shows the individual jab given by xx doctor at xx clinic.

          i brought the clinic’s letter with all the jabs stated on it but in the end the admin staff took my healthbook record so it depends on the school too but as long as all jabs have been done…give them copy to cut down on waiting time…my registration was done 1.5hr later at phase 2A2 though there were about 11 applicants that morning but if there are more admin staff, the waiting time will be shorten…

            Alamak, already checked my PSLE cert liao. He saw for himself what.

            Then I signed under the 'former student' column. If ChiefKiasu needs an assistant, I think my hubby can apply for it.

                  My KS & paranoid hubby has even signed up his own account here on KSP...he told me this morning. I was like, 'What ? You signed up on KSP ? For what :shock: ? That is the forum I go to, when I need to release frustrations.' Why can't you give me some breathing space ???


                  Last week when we registered daughter, the lady at the entrance asked us if we have ALL the documents and photocopied ones. If not, to proceed to bookshop to have them photocopied FIRST.
                  Having photocopied the documents beforehand really helps and saves time. We were done within 15 min, but then there weren't many parents b4 us...bcos they had to proceed to photocopy their documents.
