Don't try your luck. The cert is listed in http://www.moe.edu.sg/education/admissions/primary-one-registration/required-documents/ as one of the required documents.

Do you really want to risk having your application rejected because of the cert?

This was listed at the HPB - National Immunisation Registry

* It is important to request for a copy of Certificate of Vaccination from your Doctor when your child completed the vaccination according the Childhood Immunisation Schedule.

The Certificate of Vaccination is one of the document required for Primary One Registration.

Check around, I was told that even I missed out one jab, its ok, cos still can get the cert printed out.At most, the child will need to jab later.

This is what I thought.

May I know what kind of immunisation cert u submit since you missed out one jab?I dont think missing out one jab will deny the child for application into the school right?

Just to share. my son missed a 18mth MMR 2nd booster as he was sick and we forgot about it after that. They checked and gave that to him when he was in P1. Then at P2, he had to take another injection. So the record was mainly for them to identify if all injections are completed and if the child missed out anything, they will probably give that to him.

Just called the pd, and realised ds has completed all the jabs and will be printing out the cert for me.Lucky its in time to submit before the due date on 6th april.
