      i did this yest & thought to remind all parents here:


      donch wait till the last min. sure kalang kabok one. :lol:

      gather all required documents - both parents ics, child's birth cert, immunization certs & all letters from schools, clubs, association, church , etc (where neccessary & required) & file them or keep them all in one file & put it in a safe place - out of children in case they open up & tear, out of water in case it gt wet & tear, out of heated areas in case it got burnt accidentally, etc...

      Impt: remember where u place it. πŸ˜„

        does school accept the immunization summary printed from HPB?

        has anyone collected the immunization cert from HPB after requested online for self collection? how soon can we collect after the online request? thanks

          I never received any letter from the school. Just an email to ask for our particular only…

            Er… One question. I already have the immunization certificate from the doctor. But when I log in to HPB to view DD record, it is not updated. Is it ok or should I call the HPB to update it first?

              Have you called them? You clocked the PV hours, they should send you a letter stating that you did which you can use for P2B

                    Btw, can I check with all of you regarding the immunization cert..I can't use the health booklet? I went to HPB website but cant seem to find the link to print any certs. Can advise please? Thanks much. πŸ™‚

                        Do we need original doucments or just photocopy will do, ie: child's birth cert, immunization certs ?
