would like to check for immunisation certificates, do we bring the health booklet to the sch or do we have to print it out from the National immunisation registry website?

1 )Photocopy from the health booklet will do but please bring along the health booklet too. (Can double check against the NIR website)

2 ) Bring husband original NRIC and your original NRIC too.

3 ) Original Child BC

Optional: -

4 ) Photocopies of your child health ,any allergies,asthma etc to be attached to the MOH forms. Especially, any surgery before, etc. (As I was given a letter signed by the doctor who done for my girl Surgery whether she can PE and any drug allergies)

As I have attached mine as they will need it for her dental care in school.

(However at later date, the dental nurse called me for a Photocopy letter of my child as my girl had surgery before, maybe different school different procedure )

Mummies, need to ask if number of applicants in Phase 2B exceeds the places available, will those that stays within 1km still need to ballot with the rest of the Phase 2B? Or those staying within 1km of sch and in phase 2B will have a place?
