FYI - for rest of parents who need lost / misplace the cert..
According to the National Immunisation Registry Website,
Headlines
* It is important to request for a copy of Certificate of Vaccination from your Doctor when your child completed the vaccination according the Childhood Immunisation Schedule.
The Certificate of Vaccination is one of the document required for Primary One Registration.
* You can print the Immunisation History which is the equivalent to the immunisation certificate and it is acceptable by MOE for Primary One Registration -
I printed dd's cert today also, via the website. Need Singpass and child BC no. She has no records for DPT (D3), SAB (D3), Hep B (D2) and Hep B (D3). Is that correct or did I miss a jab? :nailbite:

My ds had the 2nd, 3rd and booster shot for Hep B. He also had the 3rd shot for DPT and Polio(SAB)
Maybe you cross-ref with your dd's health booklet records to chk. -

My ds2 is under Phase 1. I just called my school to confirm, and almost fell off my chair when told that the acknowledgement letters had been distributed to the older siblings yesterday. Ds1 had not received anything from his FT!
So... definitely good to call your school and confirm

Yes, got our SD done at MOE yesterday using the grandparents address.
