Have not installed any heater by myself but I think the water pipes are electrically earthed so any leakage should have tripped the supply

In both cases they mention exposed wire / not installed properly. I admit I never really think about all these... turning on water heater is like flushing toilet to me...

In this case, it is not a product problem. It's an installation problem.

I think most will ask the electrician to install the heater. Installation is one part, wear and tear of the heater is another part, there could be other parts which resulted the tragedy

My concern is how can we prevent it, given most if not all families hv at least one at home?

During lunch time, colleagues and i discussed this issue. For me, i shower in normal water but dd is used to shower with warm water. When i was young, i showered in warm water only when i was sick. And having a hot bath/shower is no simple task as my mum has to boil a big kettle of water to be mixed with cold water in a tub
As someone has shared, now showering or bathing in warm water is as easy as flushing the toilet.
