Just fill-up the application online with the score for each subject (we also did not have the copy of report book, but we have the marks/score for each subject). You will be required to bring the copy of report book and other certificates (if any) on the later stage, during the camp if I remember correctly. -

Sorry, I might have mixed it up with SST DSA form which requires the uploading of result slips etc.
For NUSH, just enter the marks as requested in the online form. Your child will be getting his/her report book after SA1. There will still be enough time for you to complete the online DSA form before the closing date of registration. During this time, you can make copy of the result slips, certificates etc (see list below) which will be required if your child is selected for the camp.
One photocopied set of the following:
Citizenship documents
Birth Certificate / Citizenship Certificate (for Singaporeans)
Birth Certificate, Re-entry Permit and Passport (for Singapore Permanent Residents)
Passport and Student Pass / Dependent Pass (for foreign students studying in Singapore)
Passport (for foreign students studying overseas)
P4, P5 (semester 1 & 2) and P6 (semester 1) school progress report
Awards / Scholarships / Competition certificates (where applicable)
Testimonials / Co-curricular activities records -
A record of 8630 visitors to this year's open house, an increase of more than 3000 over previous year :yikes:

Come and explore how you could get involved and engaged with your child’s school life, and co-create a joyful and meaningful experience as members of the PSG community.

Some CCAs are twice per week and some are once per week. For the twice per week CCA, may reduced to once per week or stop CCA when near exam week.
