6-7 Years
(Primary 1) Oral Sabin - 2nd Booster
MMR - Booster dose Poliomyelitis
Measles, Mumps & Rubella

do our children will be complusory given the dose from school (P1) or I'll still need to take my child to the clinic?

There is one jab given at P1 in school. I went to National Immmunisation Registry to print out the cert for my daughter...keyed in son's birth cert to check his jabs and saw that he was given a booster jab in July when he was in P1.

I thought they still rely on the health book of the kids ?

I will be bringing the Health booklet along, just in case...these are impt documents. Better bring along in case school said left out this and that at point of registration. Really bang wall then :stupid:

But MOE website very clear - no? We only need to bring the immunisation record (strange when this is all on the web and otherwise verifiable) and parents original NRIC. Don't scarly YOU be the one to bring a huge bag (reminiscient of pri school kids with very heavy school bags).

BOTH parents IC, child's immunisation aka health booklet MUST MUST.
No worries, I will count on hubby to bring all the necessary stuff. HaHa. Me super blur. Darling, bring BIG bag along to register P1 ? :laugh:

The health booklet is used to help the staff verify that you are submitting the correct one. The staff only need the immunisation page.

If you have a copy printed from the website, it has the child's name, BC no., etc. on it. You don't have to bring the health book.

Bring the health book only if you are making a copy of the immunisation page from the health book itself.

So des...my first time lah, a bit blur. :oops:

In any case, my PD gave me a separate piece of paper for DS1's immunisation record (got his stamp and chop) - I think the original one in health booklet became very messy so he decide to recreate. Laminated that version....machiam like cert.

Perhaps anyone interested to know the above NIR Website. Just key in either parents SINGPASS ID, then go under 'Check immunisation' , key in Child BC ,name and DOB.

They will bring back a form regarding to take in school or opt out before giving them the jab at later date.

However ,my girl was given the jab : SAB/IPV B2 & MMR/AMI B1 in the school during March.
