For my DD, it is $180 (in total) but she doesn't take HCL. So might be more if your child takes HCL. However, we were told to just check and sign the form. The examination fee of $180 was indicated in the form. However, DD said her teacher didn't say anything about collecting of money yet.

Just curious, who signed the form. DS had already signed the form and was so proud to show me his signature. Just verified with him and he confirms that the form teacher asked the students to sign, not the parents. Is it the say way for you?

DD did not bring back the form and she already signed it today and seems like this form only need to be signed by the respective student -
