        The answer is 1.The clue is have.Since have appeared,you'll have to use the past perfect,which is had only.

            1) waited
            2) was waiting
            3) have waited
            4) had been waiting

            B) Mr Lee ______ far when he realised that he did not lock his house door.

            1) did not drive
            2) has not driven
            3) had not driven
            4) was not driving

                      Ans : Not only did my mother scold me, but also my sibling.
                      The scold in the ans shldnt be a past tense right?

                        Yes. It shouldn't be in past tense as the verb that \"holds\" the tense in this case is 'did' thus the following verb 'scold' should be in the infinitive form.
