The series recommended below is useful as it has detailed notes on the rules of transformation. I used it for two years with students who were weak in S/T and they improved tremendously. However, it's not exhaustive. For them to get full marks in S/T, they must do their grammar drills as well, e.g. they need to be sure of how to change adjectives to nouns and vice versa.
