must we get a doctor’s signature to certify ?

which are the 3 compulsory injecton every child must have ?

I remember need a certified copy. Mine think is from Polyclinic. Sorry, details can't remember as it's some yrs ago.

The option to print is on the left. You will need your SingPass.

Here's the list of vaccination recommended to be completed before P1.

The printed copy from the NIR is sufficient. No need to re-certify. It has to be certified before it can be stored in the Registry so just print it out will do.

When my child who was 5 last year, I brought her for PNC B1 (Pneummococcal) jab. This is the 5th (latest) Pneummococcal jab that is recommended but not compulsory. In addition, MMRV D2 (Measles, Mumps, Rubella and Varicella Vaccine) the latest jab was taken together with Pneummococcal jab. That means my child does not need to take any jab when in P1 next year. :boogie: The Certificate of Vaccination was issued last year too. Was advised to keep it and use for P1 registration.
Please check with your paediatrician as my child has yearly check up with her pd and she will advise us accordingly on the necessary jabs to take.

I didn’t print out any immunisation schedule or cert as was advised by MOE, the primary school in question, as well as paediatrician that a copy of the child’s health booklet showing the pages of all vaccinations taken (including compulsory ones) is good enough. So I submitted just that. Just write child’s full name and BC no. on top. MOE Pupil Placement Officer checked documents and said all was intact.

I was looking at Yangzheng Pri but my distance from the sch to my hse is more den 2km.. n im nt sure whether is the chances of getting is high.(which we will be in phase 2c)

now i so worried whether will they be any balloting anot.. :nailbite:

For Yangzheng Pri, you have to monitor the situation for this yr.

There was balloting 1-2km in 2011 and balloting > 2km in 2012.

Thank you very much, to each of you, for kind info, regarding how to print out immunization record.

Later lunch time today, I am going down health promotion board (level 5) at 3 second hospital avenue (telephone : 6435-3500) - to talk to them and collect the print out on the spot, for the summary list of injections my kid had taken. I had problem with my sing pass account, so I need to go down. And the school I’m registering my child - they need this print out. They don’t want the baby booklet.
