The one that i have, doesn’t say it is an immunisation cert. Only say Immuisation History… Anyone? -
Thanks for that. Been wondering.. MOE onli say registration form but did not specify where to get the form. Alrite, then guess we will need to bring IC/ Birth cert/ and immunisation cert... for SC.
