Also, how do we void the SD should we decide not to use it? Through the school??

if go earlier 8 am, less crowded. After settle, can go home.

Checked my girl's health booklet...she's had her Hep Bs and DPT (D3) and the doc crossed out for polio, assuming coz it was administered as a 5-in-1. Anyways, should I go to the doc to update her records or can I just bring the health booklet instead?Gumbokins:
I printed dd's cert today also, via the website. Need Singpass and child BC no. She has no records for DPT (D3), SAB (D3), Hep B (D2) and Hep B (D3). Is that correct or did I miss a jab? :nailbite:
My ds had the 2nd, 3rd and booster shot for Hep B. He also had the 3rd shot for DPT and Polio(SAB)
Maybe you cross-ref with your dd's health booklet records to chk. -
