May i know how do you get from the polyclinic? They have all the records even my kid did not take the jab in polyclinic? My is from private clinic , and there is a booklet where the doctor will chop... would that book be good enough as immunisation report?Anyone has the immunisation report sample? Thanks

I saw that child immunisation report is needed for the registration?? Anyone has succesful printing from the web? I tried buy cant.... it says reports has discrepancy ...

yes that will be enough, my son had his jabs taken overseas, and they are in the overseas format. as long as the type of jab is readable is ok. Just make a photocopy of those pages from the book.
I have foreigner colleagues who used their home country format they are fine too.
It doesn't need to be in the NIR format. -

Thanks[/quote]u can go back to the clinic to get it .. .. the doctor will issue a cert with details of the vaccine taken

If you do not get a place in P2C, you will automatically be in P2CS. If you still do not get a place by P2CS, then you will be leaving it up to MOE to allocate the school for you. The key (and probably only) considerations that MOE will take into account will be gender, race and distance. So try not to leave it to fate (or MOE) to decide.one question , assuming i am not allocated a slot in my choice school phase 2C i will move on to 2Csupplement right ?
what if it is still not allocated ? MOE will allocate right ? wonder will MOE taking into consider my family has 2 kids ( Boy and gal) and allocate my son to a mixed school so that in future the sister can register in diff phase ? -

You can only get the forms from the school on the day of registration.
The forms are stamped with the name of the school, so you canβt use
it at another school. -
Hi all! Have a qns n hope someone can help me with this
I am torn between enrolling my boy in st hilda's pri at tampines and Waterway pri (new sch) at Punggol.
St hilda's is my alma mater and i have priority enrolling him. However, due to his anxiety disorder i may put him in Waterway pri which is a new sch n just beside our new house coming next yr.
Any idea if i can register my boy at st hildas under phase 2A2 (12 july) first, then if we are guaranteed a place there, am i still able to proceed to Waterway pri (phase 2c wed 27th july) to register my boy if i find Waterway may be a better school for him? I'm gonna speak to counsellors from both schools and can only decide from there.

After Phase P2CS, it will be at Phase 3. If you are Singaporean or PR, you go the schools which still have space at Phase 3 and apply directly at the school of your choice, you will still have priority over the foreigners. You do not have to leave it to MOE to allocate the school to you. Unless you still don't get a space in Phase 3, then MOE will allocate the school.
Read the bottom of Phase 3 in MOE website. -

You only get the form on the day you register.

That said, I would be curious why you would do that if the concern is over the suitability of the schools for your child. Between 2A2 and 2C, you have 15 days to do your groundwork but between now and July, you have around 2 months. What could be established in 15 days but not in 2 months?
